Schedule your initial appointment for an Utah medical cannabis card here. Payments will be IN PERSON via Credit Card, cash, Venmo. Your card information will be saved on file at check out for potential cancellation charges. You will not be charged at checkout. Charge will only occur if you fail to comply with cancelation policy. Cancellation Policy

I agree to the cancellation policy and consent to MMSLC storing my card for charges related to the cancellation policy. Policy as as follows: Cancel or reschedule 24+ hours in advance for no charge. 12–24 hours: 25% fee 6–12 hours: 50% fee
